How to Order an Exam
- Order Form: A single, combined order form is required for both PFT and EKG/ECG exams. (Please ensure you are using the updated standard form).
- Submission: Fax your completed order form to (808) 491-0300.
- Authorization Requirement: All orders must include valid ICD-10 codes for processing. Upon receipt, orders will be reviewed for diagnostic code validity. If valid, our team will contact the patient directly to schedule their appointment.
Scheduling
- Pulmonary Function Tests (PFT): Wednesdays and Thursdays, 8:00 am – 5:00 pm
- EKG / ECG: Mondays, Tuesdays, and Fridays, 8:00 am – 5:00 pm
- Appointments Only. Strictly no walk-ins accepted.
Equipment & Quality
Our diagnostic suite utilizes shared exam room spacing optimized for precise patient monitoring and testing. PFT equipment is operated by a dedicated Pulmonary Function Technician on specified service days to ensure highly accurate, high-quality physiological reporting.
